Just order as if you were paying online with our online shopping cart. Enter the type of jewelry you would like, pendant direction, bead type, size, and personal inscription.

If you live in Vermont please remember to check calculate sales tax box and then press the recaculate button.

Please print this page and send it with your picture (labeled with inscription) and a check or money order made out to "Elemental Memories". Please email us with subject line "Ordering by Mail" at info@em-jewelry.com for our mailing address.

**Other information to include... your name, phone number, address, and email address. Also indicate if you would like your picture cropped and how (ie...if you would like just the faces or to include faces and bodies and/or background).
